Why would a patient with Type 1 diabetes mellitus, fail on oral sulfonylurea (anti-diabetic) therapy?
Putting a patient with Type 1 DM on oral sulfonylurea will not work for the patient because, this class of drugs works by binding to ATP dependent potassium channels on beta cells which block potassium efflux from the cell, and the build-up of potassium plus calcium influx causes depolarization leading to insulin release. On the other hand, T1DM is caused by a total destruction of beta cells (an autoimmune disease), and so these drugs can never work for a T1DM patient since they act on beta cells.
